If you would like to add your name to the allotment waiting list, please complete the online form below.

Alternatively, you can download and print the form and send it to:

NOTE: When you are given an appointment for viewing, please provide two forms of identity as follows:

  1. Photo ID, such as driver’s license or passport
  2. Proof of address such as a recent utility bill, showing your name and home address
